Re:        Order form for materials to create a successful display board

Our CDMS Science Fair is scheduled for Friday, February 29, 2008.

As a service to you, I will be placing an order for prepaid display materials.

Please return this order form and money (make checks payable to CDMS) in an envelope to your Science teacher no later than 3:00 p.m. on, February 8. Please specify the color/s you want. Kit items will be all the same color you choose.

 Materials may be purchased at a retail store, but prices may be higher and color 

 choices are sometimes limited.
