Welcome
The Chuckey-Doak Middle School teachers
and staff welcome you to our school.
Established in July 2004, we are striving
to be the best middle school in Tennessee.
Our vision is to build student character
through leadership, guidance, and education,
and to empower our students to be good citizens.
At Chuckey-Doak Middle School, we have a

Discipline, in order to be effective, must be consistent, timely, and fair.
There is a correlation between academic success and attendance.
All students have the right to learn.
Each student is a valued individual with unique physical, social, emotional, and intellectual needs.

Chuckey-Doak Middle School's cafeteria uses
a computerized system that tracks student's purchases at breakfast and
lunch.
This system allows for the following features:
Assignment Notebook
Assignment notebooks are issued to each
student at CDMS and is paid for out of fee money.
The purpose of the assignment notebook:
Communication
Newsletters inform students and parents
of upcoming events, important activities, and key issues.
The newsletter is sent home with students
at the first of every month.
A calendar and a menu are also sent at the
same time.
CDMS has a website that has important information
and dates.
You may contact any teacher or administrator
via email.
Grade cards are sent home every six weeks,
and interim reports are sent home at the middle of each six weeks
(except for the final one). We encourage
parents to schedule conferences with teachers as needed.
Fees
A fee of $15.00 is assessed to every student.
This covers the assignment notebook, the lock rental,
science lab fees, instructional materials,
and workbooks.
Students are responsible for damaged or
lost books and must pay for their replacement or repair.
Forms are available in the office for families
who may qualify for fee waivers.
Change of Address/Phone
Please notify the teacher and school office
of any change as soon as possible.
We will make this change on the green card
and in the computer.
It is especially important that we have
emergency numbers to call when your child is sick or hurt.
Medication
We must follow the Greene County School
Board policy concerning medication to be given at CDMS.
Here are some things for you to remember:
Before both prescription or non-prescription
medication can be administered, the school must have a Permission for Medication
Form filled out. These forms are available in the school office.
For short-term medication (less than 4
weeks), a doctor's signature is not necessary on the Permission for Medication
Form, but a parent's signature is mandatory.
All medication (whether it is prescription
or non-prescription) must be in the original bottle or packaging.
Medication cannot be sent home on the
bus.
If your child's medication says to administer
3 times a day, please do this before school, after school, and at bedtime.
Dress Code (Greene County
School Board Policy)
A partial list of guidelines include:
Shirts, blouses or sweaters, pants/dresses and shoes must be worn at all times.
The practice of sagging is considered to be gang related symbolism and will not be tolerated.
No head apparel, including caps, hats, headbands, bandannas, toboggans, or hair stockings will be worn in the building.
Clothing shall fit properly and shall not be unreasonably tight or unreasonably baggy.
Pants must be worn at the waist. No sagging allowed.
Shorts cannot be any shorter than below the extended finger tips when standing.
Skirts and dresses cannot be any shorter than 2 inches above the knee. Dresses must completely cover the shoulders.
No spaghetti straps, tank tops or muscle shirts, are permitted. Bare midriffs, bare shoulders or backs, and excessively low cut clothing, along with clothing made of see-through material is prohibited. The midriff area must be fully covered at all times.
Hair styles, unusual hair color, sideburns, and mustaches must be in good taste, clean and well groomed.
Clothing that exhibits written, pictorial or implied references to illegal substances, drugs, or alcohol, negative slogans, vulgarities, or that which attracts undue attention is prohibited.
Prohibited items include:
(1) long, large and or heavy chains
(2) studded or chained accessories and
(3) sunglasses, except for health purposes.
Leotards, body suits, biking or jogging shorts, and skintight outer materials such as spandex are not appropriate.
For the safety of the school population, trench coats and dusters will not be allowed.
Facial jewelry shall be limited to ears only.
School Closings
Check your local radio stations, local television
stations, and the Internet for school closings and early dismissals.
Be prepared. Make sure your child
and your child's teacher know what to do in case of early dismissal.
Make sure you update your child's early
dismissal plan when necessary.

Students will be allowed to use the telephone
only
in case of an emergency.
Arrangements for after school activities
should be made before coming to school.
Parents are encouraged to call their child's
teacher at a time when classes are not in session.
Teachers will be called to the phone only
in emergency situations.
Please leave a message with the school's
secretaries and the teacher will get in touch with you as soon as possible.
It is extremely important for us to have
a telephone number on the registration cards so that we can reach you in
case of an emergency. If you do not have a telephone, please use
the number of a close friend, relative, or neighbor.
Also consider allowing your home number
to accept anonymous phone calls.
Some of our lines are blocked so that only
the main CDMS line shows up on caller i.d. boxes.
Visitors
We ask that all visitors register at the
school office and wear a visitor's badge throughout your time at school.
If you need to see your child's teacher,
we ask that you call in advance and schedule a conference.
Leaving School
Parents arriving to pick up children before
dismissal are required to come to the school office to sign out your child.
The school must have a note on record or
a phone call before we will release any child to anyone except an authorized
person.
If a student leaves before 11:30 a.m., he/she
will be counted absent for the school day in accordance with attendance
laws.
A student must be present the majority of
the school day to be counted present.
Outside Pick-Up of Students
All students being picked up at Chuckey-Doak
Middle School must leave through the front entrance and be picked up in
the front drive. Parents will not be allowed to wait in the hallways
or wait in the area where buses load.
School dismisses each day at 2:55 (2:30
on Fridays).
Pick-ups will exit the front of the building
at 2:50 (no student should be left after 3:10 or 2:55 on Fridays).
In order to ensure safety for all, we ask
that you follow these guidelines:
You must be in one of the two lines in
the front driveway in order to pick up your child.
Do not leave your car to pick up your
child. This causes traffic congestion.
Outside Pick-Up Rules for
Students
Please keep all articles inside your backpacks
and lunch boxes.
Food or drink items will not be allowed
outside.
Under no circumstances should a student
get in a vehicle that does not belong to their parent, guardian, or designated
ride.
Students must wait for permission to leave
the sidewalk area and get in the vehicle.
Students must walk at all times.
Grades at CDMS
Grades at CDMS are based on the Greene County
Schools Policy.
It is very important that students make
studying at home a priority at the middle school so they do not fall behind.
We are preparing them for high school and
beyond and good study habits are essential along with good attendance.
Grade cards are sent home after every six
weeks grading period.
Interim reports are sent home at the middle
of the six weeks grading period.
Grading Scale
A = 93-100
B = 85-92
C = 75-84
D = 70-74
F = 0-69
S = Satisfactory
U = Unsatisfactory
Free tutoring is available at CDMS on Tuesdays
and Thursdays from 3:00-4:30.
Forms are available in the office and should
be in your registration packet as well.
Attendance Policy
Regular school attendance is a key to good
school grades, overall achievement, and a rewarding educational experience.
Tennessee State Law also requires it.
Students are expected to be in school except
in cases of emergency or for reasons as explained below:
Personal illness - The school requires
a statement from the child's physician if he/she misses more than 3 days
in a six weeks' period.
Illness in the family - All schoolwork
should be kept current.
Observance of a religious holiday - Any
student of any religion shall be excused if his/her absence was for the
purpose of observing a religious holiday consistent with his/her creed
or belief.
Any other reason must be excused by the
school prior to the absence.
Students who miss a whole day of school
are not to participate in after school activities such as ball games
or scouts.
Steps to follow when absent include the following:
Parents should call the school (787-2038)
by 8:30 a.m. each morning.
Have parent or guardian write an excuse
giving your name, date, days of absence, reason for absence, and his/her
signature.
A doctor's excuse is necessary if more than
3 days in a six weeks' period have been missed. Otherwise it will be unexcused.
Present your excuse to your homeroom teacher.
Ask all teachers for make-up assignments.
Assignments that are not made up will be reflected in your grade.
Each student is responsible for seeing
that make-up work is completed.
Parents may call the school office and
ask that homework be brought to the office for pick up or that it be sent
home with another student. Please call in advance if you desire
to pick it up.
Five unexcused absences will result
in a note to parents advising them of the situation. Ten unexcused
absences will result in truancy hearing.
Tardiness
Promptness to class is very important.
Students are to be in their seats and ready to work when the bell rings
at 7:55 a.m.
The tardy bell rings at 8:00 a.m.
All students arriving after the tardy bell will be expected to sign in
at the office, noting the reason for being late. The third day tardy will
earn a student one lunch detention.
Three lunch detentions in a 6 week period
will result in an after school detention.
Lockers
Each student will be issued a locker for
his/her personal use. Students will rent a locker from the office
at the beginning of school. The homeroom teacher will have a copy
of the combination for use in emergencies. Lockers are subject to
search by school personnel if just cause has been presented. Lockers are
school property and are to be used properly and appropriately. Students
are responsible for all locks that are damaged or lost ($5.00).
Articles Prohibited
Articles that are disruptive to the educational
process (such as cell phones, water guns, radios, cassette/cd players,
gum, balloons, firecrackers, toys, etc.) are not to be brought to school.
Such articles will be confiscated and the offender will face appropriate
disciplinary action. Other items that cannot be brought to school
include drugs, alcohol, tobacco products, guns, pocketknives, etc.
These items are strictly prohibited on campus and will result in immediate
disciplinary action. Please visit the Greene County Schools website for
further information about zero tolerance.
Discipline
The Greene County Board of Education believes
that the school community, including students, parents, teachers, administration,
and the Board, must work together to provide a safe, supportive environment
that promotes teaching and learning. Parties shall engage in collaborative
efforts that instill a sense of self-discipline in students and that protect
the rights of individual students. Students must be taught to respect
the person and rights of others and should be courteous and cooperative
with school staff and other students.
The Greene County School Board has approved
a discipline procedure for all elementary schools.
It focuses on Levels I-V based upon severity:
Level I Behaviors
Minor student misbehaviors, which impede
orderly classroom procedures or interfere with the orderly operation of
the school, are usually handled by an individual staff member, but sometimes
require the intervention of other support personnel. (Examples
classroom or school disturbances, aggressive behavior, classroom tardiness,
cheating and lying, non-defiant, failure to do assignments or carryout
directions, harassment)
Level II Behaviors
Misbehaviors whose frequency or seriousness
tends
to disrupt the learning climate of the school; do not represent a direct
threat to the health and safety of others but are serious enough to require
corrective action on the part of administrative personnel; usually result
from the continuation of Level I misbehaviors. (Examples School
tardiness, school or class truancy, using forged notes or excuses, disruptive
school or classroom behavior, uncooperative behavior, abusive language,
inappropriate gestures, possessing pocket knife, possessing over-the-counter
drugs, using or possessing fireworks, leaving class/campus without permission,
possessing tobacco, possessing cellular phone/paging device, continued
Level I behaviors)
Level III Behaviors
Acts directed against persons or property
but whose consequences do not seriously endanger the health or safety
of others in the school; often result from the continuation of Level II
misbehaviors. (Examples Fighting, vandalism, stealing, threats
to others, severe abusive language, serious harassment, distributing over-the-counter
drugs, other acts of aggression, continued Level II misbehaviors)
Level IV Behaviors
Acts which result in violence to anothers
person or property or which pose a threat to the safety of others
in the school; may be criminal and require administrative actions which
result in the immediate removal of the student from school, the intervention
of law enforcement authorities and action by the school board. (Examples
Death threat, extortion, vandalism, theft, possession/sale of stolen
property, arson, serious harassment, other acts of violence, unmodified
Level III behaviors)
Level V Behaviors
Acts of zero tolerance, including, but not
limited to, unlawfully furnishing/selling/possessing unauthorized substances,
possession of a gun or dangerous weapon with intent to harm (one year expulsion,
which may only be modified by the director of schools and determined on
a case-by-case basis) and assault/battery.
Chuckey-Doak Middle School
will use lunch detention, after school detention, corporal punishment,
loss of privileges, or other action oas deemed appropriate by the principal
or vice principal as a consequence for Level I, II, and III behaviors.
Three lunch detentions in a 6 week period
will result in an after school detention.
In-School Suspension along with out of school
suspension will be consequences for Level II, III, IV, and V
behaviors or when deemed appropriate by
the Principal.
Fighting at CDMS will result in an automatic
3 days suspension for the first offense,
4 days suspension for the second offense,
and 5 days suspension or expulsion for the
third offense.
CDMS is a Bully Free School and will strive
to create a safe and secure learning environment for all students.
Bullies will not be tolerated at CDMS.
Extracurricular Activities
at CDMS
Students will have opportunities to participate
in extracurricular activities at Chuckey-Doak Middle School if they meet
the academic and behavior requirements established by the Greene County
School Board Policies and Chuckey-Doak Middle School Policies. Sports
will include football, volleyball, cross country, cheerleading, basketball,
baseball, and softball. Band will be available for all students in
grades 6-8.
Clubs are also be available for students.
They include Drama Club, 4-H Club, Chorus and First Priority.
Other clubs and organizations will be added when interest and opportunity
arrives. CDMS will participate in Round Robin, Science Fair, Spelling Bee,
and History Bee. The talent show takes place in May with categories for
singing, instrumental, and dance.
Parent Teacher Organization
Chuckey-Doak Middle School has a PTO which
is comprised of parents, teachers, and community members
who want to improve and support CDMS.
This club meets once a month on the second Tuesday at 6:30 p.m..
Projects include sponsoring students on
field trips, buying needed equipment for the school, buying instructional
supplies, and sponsoring the Academic Pep Rallies that we host at the end
of each six weeks grading period.
Booster Club
A Booster Club was formed to support the
athletic programs at CDMS.
They meet at 6:00 p.m. on the second Tuesday.
All parents/guardian of athletes are encouraged
to attend and help work the activities.
Band Boosters
With the implementation of a band program,
a Band Boosters was formed to assistance with
repairs, purchases, music, and concerts.
The club meets once a month with the band director
to coordinate efforts and needs. If your
child is in the band, please support this organization.
Field Trips
Field trips are an important part of the
educational process and must be earned by the student
demonstrating responsibility and respect
for the rights and needs of others.
Students will be expected to participate
in these trips and to follow all rules and direction of adults.
A student may lose the privilege of a field
trip if behavior is not appropriate.
We will work to raise money to off set the
costs of trips, but it is wise to start saving now.
The PTO and Booster Clubs also sponsor students
who are in financial need and meet certain requirements.
These trips are board approved and offer
a great opportunity for students
to experience science, social studies, and
history first hand.
School Pictures
School pictures are taken by a professional
photographer in the fall and spring.
Sports pictures are also taken at this time.
A note will be sent home explaining choice
options, photo dates, and payment procedures.
Yearbook
CDMS sells a yearbook so students can have
a memorabilia of middle school.
Fall pictures are used for class sections.
You can reserve your copy with a down payment this fall.
Yearbooks usually arrive in May.
Special Occasions
Due to the fact that glass is not permitted
at school or on the bus for safety reasons,
we ask that flowers, etc. be sent to the
child at his/her home for birthdays and holidays.
